Mildred Diaz, MSW

Mildred Diaz joined Terra Firma as the first Patient Navigator in August 2020. Before her arrival, she was a Care Manager with Montefiore (UBA) University Behavioral Health providing clinical services at all levels including medical, mental health, and substance abuse care. Mildred has over 16 years of working in the social services field in various capacities and with different populations. Mildred graduate recently in May 2022 with her Master in Social Work.
